Here is a way to submit your nominations for the 2009-2010 JazzHouston online poll based on categories. All submissions will be anonymous. Simply click the link below and fill in the spaces.

Be sure to include artist name and album title for album of the year category.

You may submit as many nominees as you like, but please only submit them once. If you run out of spaces, just take the survey again and add the different entries you wish on the next ballot.

Nominations will close Sept 12 so the final ballots can be assembled and voting can commence for awards.
